Discrimination is against the law

Afshar Medical Group does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex. We follow Federal civil rights laws that protect you from discrimination in health care.

Who this Notice applies to

This Notice applies to all of our health programs and activities, including office visits, lab and imaging coordination, and communication with our staff, whether in person, by phone, or online.

We provide reasonable modifications and free aids and services to help people with disabilities communicate effectively with us.

Language assistance for people with limited English

We provide free language assistance services to people whose primary language is not English. This may include qualified interpreters and information written in other languages to help you understand your care and your bills

Help with grievances & non-retaliation

If you need help filing a grievance, our Civil Rights Coordinator is available to assist you. We will not retaliate against you for filing a grievance or a civil rights complaint.

Filing a complaint with HHS Office for Civil Rights
