Insurance & coverage

Insurance plans

Afshar Medical Group is in-network with many major insurance plans. Because networks and benefits change, please call the number on the back of your insurance card to confirm that Afshar Medical Group and Farhad Nassiri Afshar, MD are in-network before your visit.

Copays at the time of visit

If your plan has an office visit copay, it is due at check-in. The amount is set by your insurance plan, not by our clinic.

Deductibles & coinsurance

Some visits and tests are applied to your deductible or billed with coinsurance. After we submit the claim, your insurance will process it and may assign part of the allowed amount to you. Any remaining patient responsibility will appear on a statement from our office.

Self-pay / no insurance

If you do not have active insurance or prefer to pay out of pocket, we offer self-pay visits at transparent rates. Payment is due at the time of service. Please call us before your appointment for current pricing.

Labs, imaging & outside bills

Lab tests and imaging studies are billed by outside facilities such as Quest, LabCorp, or radiology centers. Those charges are separate from our office visit fee, and you may receive additional statements directly from those providers.

Statements & payments

After your insurance has processed a claim, you may receive a statement from Afshar Medical Group for any remaining balance. You can pay securely online through our Patient Payment Portal or by phone.

Payment methods

We accept major credit and debit cards and most HSA/FSA cards. For questions about other forms of payment, please call our office.

Cancellations & no-show policy

To keep appointment times available for all patients, we ask that you cancel or reschedule at least 24 hours in advance whenever possible. Late cancellations or missed appointments may result in a no-show fee.

Financial difficulties

If you are experiencing financial hardship, please contact us before your visit or when you receive a statement. We will do our best to review options with you.

Questions about a bill

If something on your bill or Explanation of Benefits (EOB) is unclear, call our office and have your statement or EOB in front of you. We can walk through line items, clarify coding, and help you understand what your insurance covered.
