- Home blood pressure readings if you track them.
- Outside records: labs, imaging, specialist notes if available.
- Family history updates (new cancers, heart disease, diabetes).
What we typically cover
- Vitals and measurements: blood pressure, weight/BMI.
- Sleep, stress, nutrition, exercise, alcohol/tobacco.
- Screening plan (cancer, metabolic, cardiovascular risk).
- Preventive care review based on age, history, and risk.
- Current treatment and supplement review when relevant.
Common screenings (examples; individualized)
- Blood pressure screening and confirmation when needed.
- Diabetes/prediabetes screening for eligible adults.
- Colon cancer screening beginning at appropriate ages/risk.
- Women: cervical and breast screening planning based on guideline age ranges.
- Men: shared decision-making on PSA screening when appropriate.
When to book sooner than “annual”
- Chest pain, shortness of breath, fainting, blood in stool/urine, severe headaches, unintentional weight loss, or major mood changes should be evaluated promptly.
